Your gift card is earned when the person you refer comes in and completes screening.
Does the person I refer have to qualify?
Yes. They need to be 75 or older, live in assisted living, and still need a seasonal flu vaccine. Additional eligibility requirements apply. Denali Health confirms eligibility during screening.
Is transportation provided?
Yes. Transportation is provided for the participant’s clinic visit. No insurance is required.
